CIRCLE OF CARE, CLIENT SERVICES WORKER

Position Summary

The position of Circle of Care, Client Services Worker requires the delivery of foot care clinics and events.  The position involves liaising with and the coordination of foot care clinics hosts, clinicians, Indigenous practitioners, community partners and clients as well as interdepartmental networking within the organization.

Responsibilities and Duties

  • Assist in the coordination and implementation of SOADI Circle of Care events throughout Ontario
  • Preparation of event equipment and resources
  • Manage, approve and process orders through the subsidy program
  • Perform administrative duties, data base input, calendar updates, quarterly activity reports
  • Monitor client files while maintain file safety and client confidentiality
  • Coordination and facilitation of head office ongoing foot care clinics
  • Assist with Healthy Lifestyle Fridays; coordinate client appointments
  • Update SOADI social media and website with relevant event information
  • Set-up of event clinics, liaise with host, registrations, evaluations, taking down of clinic, travel
  • Attend team meeting as required
  • Represent SOADI at all functions in a professional manner and with respect to our mandate, mission statement and values
  • Perform other duties and responsibilities that may be designated by the Circle of Care Coordinator

Supervision: The Circle of Care Client Services Worker will be under the supervision of Circle of Care Program Coordinator and report directly to the Coordinator and ED/PD.

Qualifications:

  • University/College Degree and/or 2+ years’ experience
  • Established and proven record as a dependable and competent person
  • Working knowledge of office technology, using a laptop computer for word processing and communications
  • Knowledge of Indigenous Organizations
  • Knowledge of diabetes and foot care, especially pertaining to Indigenous population, health background a definite asset
  • Co-operative, conscientious, and reliable, with strong skills in planning and work organization
  • Able to travel and has a valid drivers’ license
  • Able to lift and transport large display boards and resources

Location: SOADI Head Office 3250 Schmon Pkwy Unit 1B, Thorold, ON

Salary Range: to be determined based on experience

Employment Period/Status: Begin immediately – March 31, 2017, contract position
